The search for the 2021 Health Equity Heroes has officially begun.

For the past six years, the DentaQuest Health Equity Hero awards have celebrated individuals and organizations behind extraordinary work that champions equitable oral care and health care. In 2020, our 12 Heroes provided 12 inspiring bright spots in a challenging year for everyone. For 2021, stories providing hope and optimism remain important as we continue to adapt and cope.“Our annual call for Health Equity Heroes is a refreshing spotlight on exceptional work addressing deep-rooted disparities, even in the midst of the coronavirus pandemic,” said Steve Pollock, president and CEO of DentaQuest. “These Heroes are community leaders whose creativity and actions inspire us to work harder and do more on behalf of those with the greatest needs and the fewest resources.”

Nominees can come from any sector, including dental care, health care, family support, legal aid, schools and community programs.

And anyone can nominate a Hero! Fill out and submit this form before April 30. DentaQuest will announce the winners in June.

DentaQuest will choose 12 Health Equity Heroes for 2021. Each will receive:

• a commemorative plaque, highlighting their accomplishment,
• a $5,000 donation to a nonprofit — the Hero gets to choose — that is committed to promoting health equity,
• recognition in DentaQuest outreach and communication materials throughout the year.
